Job summary

Connections Health Solutions is seeking a Case Manager to oversee care for youth and adults in crisis, coordinating admissions, treatment planning, and discharge. You will work with a multidisciplinary team to ensure safe, person-centered plans and effective referrals, while documenting activities and maintaining compliance with PA behavioral health processes.

The role requires a behavioral health degree or substantial experience, background checks, and adherence to vaccination policy.

Qualifications

• Bachelor's degree in behavioral health or related field or 4 years of behavioral health experience.
• Knowledge of the Pennsylvania State behavioral health system of care and resources within it.
• Must pass the following background checks: Child Abuse Clearance, FBI Criminal Background Check, Pennsylvania State Police Criminal Record Check.
• Company has a mandatory vaccination policy and related requirements for employment.

Responsibilities

• Admission & Consent: Facilitate admission intakes, informing individuals and families about consents for treatment and authorizations for use and disclosures.
• Treatment Planning: Collaborate with the Interdisciplinary Treatment Team to develop and implement individualized Treatment Plans; review with individuals and families.
• Collateral Gathering & Assessment: Gather collateral information to assess needs for treatment and discharge planning.
• Care Coordination: Actively coordinate care and provide referrals to align services with facility staff and external providers.
• Discharge Planning: Engage with the team to monitor and communicate with individuals and families during discharge.
• Documentation: Maintain accurate clinic records of interventions, coordination of care, discharge planning, and treatment planning.
• Community & System Knowledge: Maintain knowledge of PA behavioral health system, funding sources, 302 processes, and resources for youth and adults.
• Professional Development: Participate in ongoing education, training, and competency assessments.

Education

Bachelor's degree in behavioral health or related field
