Job summary

Qualifications

• Requires a High School Diploma or GED; Bachelor's degree preferred.
• Minimum 3 years of case management experience.
• Strong verbal and written communication skills required.
• Ability to work under stress and irregular hours.
• Competency in using electronic health records (EHR) and telemedicine tools.
• Background checks including clean MVR are required.

Responsibilities

• Act as liaison between therapist, care team, client and family to coordinate care.
• Arrange family visits and transport; ensure client needs are met.
• Maintain clear communication with family, referrals, and Path Behavioral Healthcare.
• Attend court hearings as appropriate.
• Ensure documentation and billing adhere to policy and compliance standards.
• Coordinate and monitor client care within the ISP and community services.
