Pathways Inc. seeks a Targeted Case Manager for the West Liberty, KY area to provide comprehensive case management services to consumers, coordinating behavioral health, medical, social, and community resources.

You will maintain documentation aligned with regulatory requirements and collaborate with families, providers, and the care team to ensure seamless care. The role requires a Bachelor's degree in Human Services or a related field and at least one year of post-graduate experience;

Qualifications

• Bachelor’s Degree in Human Services or a related field required.
• Minimum of one year of post-graduate experience required.
• Experience in behavioral health preferred.

Responsabilités

• Provide case management services to assigned consumers in accordance with individualized treatment and service plans
• Coordinate and monitor services to support consumers’ behavioral health, medical, social, and community resource needs
• Maintain accurate, timely, and current documentation in compliance with agency, state, and federal requirements
• Collaborate with consumers, families, providers, and community partners to ensure continuity of care
• Assist consumers with accessing community resources, benefits, and supportive services
• Participate in treatment team meetings and contribute to care planning efforts
• Adhere to all Pathways policies, procedures, and confidentiality standards
• Perform additional duties as assigned within the scope of the position
