Job summary

The Wright Center for Graduate Medical Education is seeking a COE Case Manager to provide comprehensive behavioral health case management for individuals with addiction and co-occurring disorders, with a focus on opioid use disorders.

The role connects clients with MAT, primary care, psychosocial services, and other supports, ensuring timely access and coordinated care while maintaining client confidentiality.

Qualifications

• Bachelor’s degree in social work or closely related field; LSW preferred.
• 3–5 years’ experience in substance abuse care.
• Current BLS certification.
• Understanding of opioid use disorders and related psychosocial issues.
• Knowledge of community health resources and public entitlement programs.
• Ability to work with diverse populations and in emergency situations.

Responsibilities

• Provide comprehensive Behavioral Health Case Management services to clients (including treatment adherence counseling).
• Develop and implement individualized service plans.
• Follow up with clients to monitor progress and access resources.
• Support clients in unstable living situations.
• Maintain client confidentiality per HIPAA and organizational policy.
• Serve as client advocate and liaison between interdisciplinary team and external services.
• Conduct client home visits as needed to accomplish service plan goals.
• Participate in team meetings and trainings.
• Assist with data collection and grant reporting.
