Job summary

Family Health Center – Southwest Indiana is seeking a Case Manager to provide coordinated, client-centered services for individuals with mental health and co-occurring disorders. The role focuses on assessments, planning, coordination, and connection to community resources.

The position is 1.0 FTE, 40 hours per week. Essential duties include comprehensive assessments, developing PCTPs, providing services, and advocating for access to housing, benefits, and healthcare.

Qualifications

• Education: Bachelor’s degree in a field related to mental health.
• Certification: N/A
• Licensure: Valid Driver’s license; must be insurable under the organization’s vehicle insurance policy.
• Prefer 2 years of experience in community resource acquisition and working with individuals with mental health or co-occurring disorders.

Responsibilities

• Conduct comprehensive assessments to determine mental health, social, medical, housing, and vocational needs.
• Develop and complete Person-Centered Treatment Plans (PCTP) and treatment plan reviews with clients and their families within policy timelines.
• Provide services to clients and their families as outlined in the PCTP including case management and individual skills training.
• Monitor client progress and adjust service plans accordingly.
• Enter progress notes, complete State Registration and assessments, maintain EHR, and complete billing documentation within policy timelines.
• Participate in team meetings, communicate with internal and external team members to meet clients’ needs.
• Meet with clients and/or their families based on the level of need and state assessment.
• Provide crisis intervention and risk assessment as needed.
• Advocate for clients’ access to services including housing, benefits, employment, and healthcare.
• Maintain knowledge of Medicaid Rehabilitation Option (MRO) guidelines for service provision and documentation.
• Follow expectations set forth by Family Health Center in training, productivity, policy, and documentation.
