Job summary

Coastal Horizons Center, Inc. is seeking a Case Manager for Medication Assisted Treatment to support the Opioid Treatment Program. Responsibilities include coordinating clinical care, maintaining ethical documentation, and facilitating group therapy.

Early morning hours are required and the role emphasizes patient support and collaboration with the medical team. Qualifications include state-specific professional registration (NCSAPPB CDACI/CADC) and a background in MAT settings.

Qualifications

• Meets requirements under 10A NCAC 27G.0104 as a Qualified Professional.
• Registration with NCSAPPB as CDACI/CADC.
• One year or more of experience working within medication assisted treatment programs preferred.

Responsibilities

• Maintains clients' records by reviewing case notes, logging events and progress.
• Monitors cases by verifying clients' attendance and urine drug screens; observes and evaluates treatments and responses.
• Communicates clients' progress by participating in weekly interdisciplinary meetings and sharing results with the team and family.
• Coordinates care with clinical and medical teams by scheduling appointments and linking to community supports.
• Maintains ethical and professional standards under supervisor direction.
• Interacts with collateral issues including finances, medical, vocational, health, and behavior.
