Job summary

The Wright Center is seeking a COE Case Manager to deliver integrated behavioral health case management for individuals with opioid use disorders. You will link clients to MAT, primary care, psychosocial services, and community resources, while supporting clinical staff and ensuring HIPAA-compliant care plans.

Responsibilities include developing client-centered service plans, monitoring progress, advocating for patients, and coordinating with an interdisciplinary team to meet unmet needs.

Qualifications

• 3-5 years’ experience in substance abuse care.
• Bachelor’s degree or higher in a related field preferred; LSW a plus.
• Knowledge of medical and psychosocial issues related to opioid use disorders.
• Experience navigating health care entities and social service agencies.
• Commitment to confidentiality and HIPAA standards.

Responsibilities

• Provide comprehensive Behavioral Health Case Management services to clients.
• Develop and implement individualized service plans.
• Coordinate with interdisciplinary team and community resources.
• Advocate for clients and ensure continuity of care.
• Complete client home visits when needed.
• Assist program with data collection and grant reporting.
