Job summary

Pinnacle Treatment Centers is seeking a COE Case Manager to coordinate connections to community services and provide ongoing case management for patients. The role emphasizes collaboration with counselors and medical teammates to maintain positive patient care experiences.

Qualifications

• Associates or better in Human Services.
• Ability to travel up to 25% locally as needed.
• Experience in case management and/or utilization review preferred.

Responsibilities

• Assess referrals for appropriate level of care and social determinants of health.
• Admit new patients to treatment programs as appropriate.
• Coordinate services with counselors, medical team, and external providers.
• Monitor cases and advocate for needed services; support patient progress.
