Job summary

Qualifications

• Bachelor’s degree in social work, psychology, sociology, counseling, or related field preferred; LSW a plus.
• 3–5 years’ experience in substance abuse care and related services.
• High school diploma or equivalent required.
• Current BLS (Basic Life Support) certification is required.
• Understanding of opioid use disorders and related healthcare systems.
• Knowledge of health care entities and public entitlement programs.
• Ability to work with diverse populations and in urgent situations.

Responsibilities

• Provide comprehensive Behavioral Health Case Management services, including treatment adherence counseling.
• Develop and implement individualized service plans for clients.
• Follow up with clients to monitor progress and connect to resources.
• Coordinate with interdisciplinary team and advocate for clients.
• Conduct home visits when needed and participate in trainings.
• Maintain client confidentiality per HIPAA and organizational policies.
• Support data collection and grant reporting as required.
