Job summary

Neighborhood Outreach Access to Health (NOAH) is seeking a Case Manager (Behavioral Health Technician) to provide clinical services aimed at improving treatment outcomes and promoting biopsychosocial health within a licensed clinician-led team.

The role emphasizes coordinating care across levels, addressing social determinants of health, and helping patients access benefits while maintaining confidentiality and high standards of care. Bilingual Spanish is preferred for broader community support.

Qualifications

• English proficiency; Spanish in addition preferred.

Responsibilities

• Coordinate care with patient, families, and providers to ensure continuity of care.
• Identify and access resources addressing social determinants of health (SDOH).
• Assist patients in applying for social security benefits.
• Support engagement and follow-up after crisis events.
• Participate in formal and informal case staffing and meetings as needed.
• Maintain patient confidentiality and comply with NOAH policies and regulations.
