Job summary

Responsibilities

• Coordinate and provide care that is safe, timely, effective, efficient, and client-centered.
• Manage case assignments, draft service plans, and review case progress regularly.
• Advocate for clients, assisting them in decisions about clinical status and treatment options.
• Collaborate with therapists, clients, and family members to ensure needs are met.
• Facilitate clients in achieving independence and developing daily living skills.
• Perform other duties as assigned.
• Upholds Documentation and Billing practices per CARF and CMS.
