Qualifications

• Strong reading, writing and communication skills.

• Working knowledge of MS Office and eClinical software.
• Experience linking patients to community resources and services.
• Associate degree required; Bachelor’s degree preferred.

Responsibilities

• Clerical and administrative tasks related to patient care, including scheduling appointments and coordinating care.
• Manage referrals and calls related to integrated care and behavioral health.
• Provide education to patients about services.
• Assist with care coordination and obtaining referrals/ROIs as needed.
• Complete screeners to identify patient needs and develop care plans.
• Support providers with patient follow-up to improve adherence.
• Participate in quality improvement activities and risk management.
• Work as part of the care team within the Patient Centered Medical Home model.
• Perform other duties as assigned.
