Medical Case Manager RN - Behavioral Health

Join our team as a Registered Nurse in the supportive housing sector, where you will work alongside mental health professionals to assist clients transitioning from hospitals to community living.

Location: Bronx, NY (Onsite)

Employment Type: Contract Mon-Fri, 09:00am-05:00pm

Pay Range: $50.00-$53.00 per hour

Responsibilities

• Complete medical and communicable disease risk assessments promptly.
• Coordinate medication delivery and manage returns with the pharmacy.
• Observe self-administration of detox medications per protocols.
• Conduct urine screenings following program policies.
• Provide health education and consultation to clients individually or in groups.
• Maintain accurate and timely documentation of medical services.
• Ensure client compliance with prescribed medical regimens.

Qualifications

• Valid NYS RN license.
