Job summary

Mono County is seeking a case manager to provide counseling and case management in behavioral health services, including mental health and substance use disorder care. You will assess client needs, develop and monitor treatment plans, and support rehabilitation as part of a multidisciplinary team.

Responsibilities include conducting intakes, evaluating progress, maintaining records, coordinating services, and connecting clients with community resources.

Qualifications

• Two years of experience providing insight into mental health, drug abuse, alcohol abuse, elders, or domestic relations.
• Upper division college courses in community resources, human services, behavioral sciences or related fields.
• Bilingual candidates are encouraged to apply.

Responsibilities

• Conduct client intakes and evaluate treatment progress.
• Maintain records and coordinate services to connect clients with community resources.
• Engage in community outreach, organize events and programs, and facilitate wellness activities.
• Represent the department in community partnerships and committees.
