Qualifications

• BA in Social Work or related field is required.
• Valid California driver's license required.
• Lifesaving Certification required.

Responsibilities

• Provide direct care to UHC patients with behavioral health problems or social service needs.
• Assist primary care providers with consultative visits through needs assessment and warm handoffs.
• Collaborate with Perinatal services for referral and treatment of psycho-social needs.
• Maintain accurate records and timely documentation of caseloads and reports.
• Coordinate with clinics and community resources to support patient functioning and access to care.
