Job summary

Community Care of West Virginia (CCWV) is seeking a dedicated case manager to oversee a caseload of adolescents and adults with long‑term mental illness and/or substance use disorders. You will assess needs, develop treatment plans, coordinate services, and work with a multidisciplinary team to help patients achieve recovery goals.

You will maintain accurate records, communicate with patients and families, and ensure timely referrals while aligning with CCWV policies and regulatory standards.

Qualifications

• Combination of behavioral health work experience or relevant education and training.
• Bachelor’s degree in social work, psychology, or human services-related field is required.
• Experience in behavioral health with adolescents and/or chemical dependency is preferred.

Responsibilities

• Manage an assigned caseload of adolescent and adult patients with mental illness and/or addiction.
• Develop and implement treatment plans in collaboration with patients and providers.
• Coordinate services and referrals, ensuring timely follow-up and documentation.
• Conduct face-to-face reviews and adjust plans to meet goals.
• Maintain accurate case records in accordance with regulatory standards.
