Job summary

Qualifications

• High School Diploma or GED required; Bachelor's degree preferred.
• Candidate must have at least 3 years of case management experience.
• Ability to work under stress and irregular hours.
• Excellent verbal and written communication; strong grammar and documentation skills.
• Proficiency with EHR systems and telemedicine tools; maintain confidentiality per HIPAA.

Responsibilities

• Coordinate care with therapists, families, and care team to ensure optimal treatment. Desks, file, and coordinate appointments and transportation as needed.
• Maintain clear, ongoing communication with families, referrals, and Path Behavioral Healthcare.
• Attend court hearings as appropriate and document services in the EHR.
• Ensure documentation, billing, and compliance with policy and standards.
• Lead and participate in team meetings; develop daily living skills and client empowerment.
