Qualifications

• High School Diploma or equivalent required.
• 2 years in a drug and alcohol or mental health setting preferred; supervisory experience desirable.
• Current CPR, AED, and First Aid certifications required.
• Valid driver’s license preferred.

Responsibilities

• Supervise and engage case coordinator staff, providing regular oversight and guidance.
• Develop and maintain staff schedules.
• Coordinate client engagement, including appointments, aftercare plans, and collaboration with the treatment team.
• Maintain professional communication with referral sources and outside providers, addressing UR, legal, and employment needs.
• Provide direct case coordination services, including transportation coordination and individual sessions.
• Assist in developing aftercare plans and facilitating referrals to appropriate providers.
• Facilitate support groups to enhance clinical services and client engagement.
• Document all client interactions in Carelogic within program timelines.
• Collaborate with the client navigation team to address aftercare needs and EOS.
