Job summary

Grace Health is seeking a Targeted Case Manager to support high-risk patients through comprehensive case management, advocacy, and resource coordination. You will collect social and economic information, develop care plans, and link clients to healthcare, housing, education, and transportation services.

The role requires strong communication, independent work capability, and collaboration with the care team to improve patient outcomes within Grace Health’s integrated model.

Qualifications

• Bachelor’s degree in Behavioral Health or related field; 3+ years may substitute for education.
• Completion of a 12‑hour Targeted Case Management training is preferred after hire.
• Strong knowledge of Opioid Use Disorder, Alcohol Abuse, and Medication-Assisted Treatment.

Responsibilities

• Provide group, individual, or family case management to clients as assigned.
• Coordinate care with community providers, hospitals, and other facilities to establish a continuum of care.
• Assist patients with re‑emergence into daily living activities and recovery goals.
• Maintain accurate and timely documentation and patient confidentiality per HIPAA.
