Job summary

Grace Medical Center is seeking a Community Care Coordinator to promote the health and welfare of assigned patients. The role involves face-to-face, phone, and email outreach as part of the Interdisciplinary Team (IDT) to address physical and social needs, linking patients with community resources and ensuring timely access to services while honoring patient wishes.

Required: 1-3 years of relevant experience; preferred experience with care managers in acute care or payer settings; EMR competency

Qualifications

• 1-3 years of experience in care coordination or related field.
• Prefer experience with care managers in acute care or health insurance/payers.
• EMR competency desirable.
• Bi-lingual preferable (market specific).

Responsibilities

• Coordinate care and identify patient needs with the IDT.
• Link patients to community resources and ensure timely access to services.
• Advocate for patients, respecting rights and wishes of patient and family.
