Job summary

Elk Ridge Community Health seeks a Behavioral Health Case Manager to join the patient care team, providing direct case management and coordinating with the interdisciplinary team to support treatment plans, discharge planning, and access to services.

Responsibilities include ensuring timely documentation, advocacy for patients and families, and navigation of community resources with HIPAA compliance to improve patient outcomes.

Qualifications

• Bachelor’s Degree in Social Work, Psychology, Sociology, Public Health or a related field from an accredited college or university.
• One (1) year of experience working in human services, behavioral health, or case management, preferably in a healthcare environment.
• Current certification in Basic Life Support (BLS) for health care providers.
• Level I fingerprint clearance card: current valid and in good standing or have applied for it within seven working days after beginning employment.

Responsibilities

• Provides direct case management to patients actively enrolled in behavioral health services, coordinating with clinical team to address needs and goals.
• Coordinate care activities to ensure continuity of care between levels of care, community resources, and health plans.
• Maintain timely and accurate documentation meeting regulatory standards and ensure HIPAA compliance.
• Advocate for patients and families, fostering collaboration with the patient care team.
