Job summary

Lighthouse Behavioral Wellness Centers is seeking a Care Coordinator to provide case management, rehabilitation and wellness services to individuals and groups. The role supports stabilization after crises or hospitalizations and coordinates care with partners.

Under supervision of the Director of Outpatient Services, the coordinator will assess needs, create care plans and document services while promoting engagement in community resources and wellness goals.

Qualifications

Bachelor or Master degree in education, psychology, social work, sociology, criminal justice, or similar behavioral health field approved by ODMHSAS
Oklahoma nursing license with behavioral health experience orBachelor’s or Master’s degree with 6 months behavioral health experience or 60 college credits + 12 months behavioral health experience
Current BH Case Manager II certification per ODMHSAS Standards and Criteria
Current BH Wellness Coach certification per ODMHSAS Standards and Criteria for BH Case Manager II

Responsibilities

Provide case management, rehabilitation and wellness services to individuals, families and groups to promote emotional or psychological change and recovery.
Provide crisis management and stabilization as assigned.
Assess needs, develop care plans, coordinate with internal/external providers and monitor progress.
Complete all agency documentation accurately and timely.
Assist clients to access housing, employment, medical, nutrition and other community resources.
