Job summary

Segue Health – Faith Based Medical Practice in Meridian, MS is seeking a compassionate Care Coordinator to join our team, coordinating post-discharge transitional care management with local hospitals and home visits.

The ideal candidate will have healthcare experience, strong organizational and communication skills, be proficient with Microsoft Excel and EMR/EHR software, and be willing to travel to local hospital sites. Full-time, Monday–Friday, 8-hour shifts with competitive benefits.

Qualifications

Experience in the medical field.
Proficiency with Microsoft Excel and EMR/EHR software.
Must have transportation to the local hospital.
Heart to serve others; patient-centered.
Attention to detail and organizational skills.

Responsibilities

Working with referring facility to generate list of patients eligible for TCM services.
Visiting referrals while in the facility to explain the TCM program and determine needs post discharge.
Daily logging of referrals and discharges.
Creating charts in EMR and uploading face sheet, H&P, and DC Summary.
Post discharge calls to patients within 24 hours.
Scheduling of patients preferably prior to discharge for in home visits.
