Responsabilidades

• Oversee complex discharge plans ensuring patients have housing, follow-up care, medications.
• Address Social Determinants of Health (SDOH) like food insecurity and housing instability.
• Provide emotional support, crisis intervention, and connect patients to behavioral health services.
• Advocate for patients and coordinate community resources post-discharge.
