Job summary

INTEGRIS Health Baptist Medical Center in Oklahoma City, OK is seeking a full-time Care Management Associate for day shift to coordinate post-acute care and discharge planning.

You will educate patients and families about home health, hospice, skilled nursing, rehab and home infusion services, and collaborate with the interdisciplinary team to ensure smooth transitions and accurate documentation.

Qualifications

• Bachelor's degree in health related field or equivalent healthcare experience.
• LPN licensure in Oklahoma or multistate license, or a health-related degree.
• BLS certification obtained within 30 days of hire.

Responsibilities

• Educates patients and families on home health, hospice, skilled nursing, nursing home, rehab and home infusion services.
• Coordinates care and discharge planning with Social Worker/RN Case Manager and other care team.
• Documents referrals in the medical record and communicates with receiving agencies and payers.
• Serves as a resource to Physicians, Case Manager, Social Worker and interdisciplinary team.
• Monitors patient status and ensures smooth transitions and appropriate documentation.
