Eskenazi Health serves as the public hospital division of the Health & Hospital Corporation of Marion County. Physicians provide a comprehensive range of primary and specialty care services at the 327-bed hospital and outpatient facilities both on and off of the Eskenazi Health downtown campus as well as at 10 Eskenazi Health Center sites located throughout Indianapolis.
As a Care Coordinator at Sandra Eskenazi Mental Health Center, you'll have the opportunity to support diverse populations across our behavioral health programs. We have current and future openings across multiple teams, and during the hiring process, we'll work with you to identify the program that best aligns with your experience, interests, and career goals. Depending on the role, services may be provided in office, home, and community settings, with varying levels of community-based work.
As a Care Coordinator, you will help individuals build skills, overcome barriers, and access the services and resources needed to support recovery and overall well-being. In collaboration with the treatment team, you will conduct assessments, develop and revise treatment plans, provide skills training, and advocate for clients to ensure access to needed services and community supports.
Care Coordinators provide services in a variety of settings based on program needs, including office, home, and community environments. Some roles may involve home visits, meeting clients in community settings, and providing transportation to support access to treatment and services. Mileage reimbursement is available for approved work-related travel.
Bachelor’s degree required
If incumbent holds a bachelor’s of social work degree, incumbent must also possess a current, unencumbered Licensed Bachelor’s Social Worker license in the State of Indiana
Valid Indiana driver’s license and driving record that meets Eskenazi Health’s driving policy requirements
