Job summary

Sandra Eskenazi Mental Health Center in Indianapolis, IN, seeks a Care Coordinator to support clients with mental health and substance use needs. You will work with a treatment team to assess clients, develop and revise care plans, and advocate for access to services within the Recovery Model.

Qualifications

• Bachelor's degree required; LBSW licensed in Indiana preferred.
• Driver's license and clean driving record per policy.
• Experience in behavioral health is beneficial.

Responsibilities

• Embrace Recovery Model and motivational interviewing in care planning.
• Provide client assessments and develop revising treatment plans.
• Advocate for clients and build community linkages and resources.
• Conduct group and individual skills training as needed.
• Ensure timely documentation and billing accuracy.
• Provide case management with families and supports.
• May provide transportation using agency vehicles or own vehicle.
