Job summary

LifeLong Medical Care seeks a Community Health Worker to coordinate behavioral health referrals, screen patients, and support integration of care. You will manage schedules, assist with screenings, and maintain resource lists for the East Bay community while engaging diverse populations.

The role emphasizes patient outreach, data collection, and collaboration with clinicians to improve continuity of care and access to services.

Qualifications

• Bachelor’s degree in Social Work, Health or Human Services or equivalent experience.
• Administrative experience in health or social service setting.
• Knowledge of East Bay health and social service resources.
• Experience with mental health, substance use, disabilities, or homelessness services preferred.
• Proficient in Microsoft Office and database management.

Responsibilities

• Facilitates and manages referrals to therapy and psychiatry.
• Screens, triages, and connects patients to behavioral health resources.
• Maintains provider schedules and optimizes productivity.
• Executes warm hand offs to integrate care and connect high priority patients.
• Supports patient panels with reminders and follow-ups to improve continuity.
• Collects data, generates reports, and supports special projects.
