Job summary

Men's Health Foundation seeks a Behavioral Health Coordinator to manage referrals, support continuity of care, and help patients navigate Behavioral Health services. This non-clinical role ensures confidentiality and coordinates with various providers to promote timely access to services.

The position requires at least 2 years in healthcare admin, strong organizational and communication skills, and proficiency with EHR/EMR systems. A flexible, patient-centered approach is essential.

Qualifications

• High school diploma or equivalent; related degree preferred.
• Minimum two years in healthcare or behavioral health admin roles.
• Experience with patient registration, scheduling, and insurance eligibility.
• Strong organizational skills and attention to detail.
• Knowledge of HIPAA privacy requirements and patient confidentiality.
• Excellent customer service and communication abilities.
• Ability to work on an interdisciplinary healthcare team.

Responsibilities

• Manage internal and external Behavioral Health referrals from receipt through completion.
• Monitor referral status and follow up with patients and providers to close the loop.
• Conduct patient outreach and non-clinical care coordination as directed.
• Assist patients navigating barriers to care and connect to resources.
• Coordinate warm handoffs between patients and Behavioral Health specialists.
• Escalate clinical questions or safety concerns to the appropriate clinician.
• Maintain documentation of referrals and care coordination activities in the EHR.
• Provide front-desk and administrative support for Behavioral Health services.
• Ensure confidentiality and HIPAA compliance in handling information.
• Perform other duties to support Behavioral Health and clinic operations.
