Job summary

MaineHealth Behavioral Health seeks a Care Coordinator - Behavioral Health to provide direct outreach and linkages to community resources for consumers in assigned programs. You will guide clients and families through the care continuum from referral through ongoing care and self-management or active treatment.

The role requires a Bachelor's in Social Work or related field, with preferred experience in behavioral health and collaboration across providers.

Qualifications

• Education: Bachelor's Degree in Social Work or related field preferred.
• Experience: Two years of experience working with behavioral health consumers and families preferred.
• Knowledge about community resources is preferred.
