Case Management, Health Education, And Benefits Enrollment.
The Behavioral Health Navigator involves implementing care plans designed by social workers to address patients’ identified needs, resources, and goals.
Key responsibilities include educating patients and their families about available options, prioritizing cases based on urgency, and effectively utilizing community resources.
The position promotes patient self-management and independence by assisting with
Essential Services Such As Transportation And Housing. Additionally, The role requires maintaining accurate documentation, participating in departmental meetings, and fostering collaborative teamwork while adhering to organizational policies and ethical standards.
The candidate must possess strong communication skills, cultural competency, and the ability to work independently and as part of a team. Knowledge of federal and state regulations, particularly regarding patient confidentiality and HIPAA, is essential.
