Job summary

Centene Corporation is seeking a Behavioral Health Care Manager to assess and coordinate complex care for members with mental health and substance use needs. This role emphasizes personalized care plans, education for members and families, and collaboration with providers.

The position offers up to 75% local travel, remote work for the remaining time, and a $5,000 sign-on bonus. Requires a Master's degree in Behavioral Health or Social Work (or Nursing) and licensure; serves NW Kansas counties

Qualifications

• Master's degree in Behavioral Health or Social Work or Nursing is required.
• 2–4 years of related experience in behavioral health or care management.
• Licensed Master Behavioral Health Professional (e.g., LCSW, LMSW, LMFT, LMHC, LPC) or RN per state requirements with BH experience is required.

Responsibilities

• Evaluate member needs via phone or in-home visits and develop care plans.
• Conduct telephonic, digital, home, or site visits to assess needs and coordinate resources.
• Develop ongoing care plans for high acuity members and identify providers and community resources.
• Coordinate between member/family and care team to ensure services are accessible.
• Monitor and adjust care plans based on treatment changes and outcomes.
• Facilitate care coordination to ensure timely access to needed services.
• Document member information and care activities to meet regulatory requirements.
• Educate members and families on procedures, options, and benefits, including BH and social determinants.
• Provide feedback to leadership to improve care delivery and cost-effectiveness.
