Responsibilities

• Apply advanced clinical judgment to facilitate member physical and behavioral health care.
• Plan and assess care, direct provider collaboration, and coordinate psychosocial wraparound services.
• Instructs utilization of available resources to achieve optimal, cost-effective outcomes.
• Coordinate schedules and resources to support 40 hours weekly with possible 2 evenings per week until 9pm local time.
