Job summary

Red Rock Behavioral Health Services is seeking a Care Coordinator to serve as a single point of accountability for accessing, coordinating, and delivering services in a strengths-based, family-driven approach. The role supports wraparound and service coordination within the Health Team as needed.

The ideal candidate will have BHCM II certification or equivalent and a background in behavioral health or RN BH experience, with strong communication and advocacy skills and a willingness to travel

Qualifications

• BHCM II certification or equivalent required.
• Bachelor's or Master's in Behavioral Health, or RN with BH experience.
• Bilingual preferred.
• Two+ years of community services/health agency experience with diverse populations.

Responsibilities

• Coordinates services including mental and physical health.
• Completes multi-level assessments within set timelines.
• Communicates with community team, review committee and others as needed.
• Advocates and links families to community resources.
• Provides crisis assistance and trainings.
• Maintains relationships with agencies and partners.
