Adult & Child Health in Indianapolis seeks a dedicated coordinator to support patients receiving Medication Assisted Treatment. You will review assessments, arrange outpatient services, and maintain cross-system communication to ensure seamless transitions in care.

Responsibilities include developing individualized treatment plans, connecting families with community resources, and providing crisis intervention as needed.

Competenze

• Graduate from an accredited college or university with a Bachelor’s Degree in one of the human service fields (social work, psychology, human development and family life, criminal justice, counseling), nursing, or education is preferred.
• Knowledge of case management and rehabilitation methods, principles and techniques as they relate to mental health and/or substance use disorders.
• Familiarity with supervisory and training practices in a substance use disorders/mental health setting; strong written/verbal communication.

Mansioni

• Coordinate outpatient mental health and/or substance use disorder services as needed.
• Review initial assessments and discharge planning to identify barriers and needs (employment, education, healthcare, social connectedness, justice involvement).
• Document all services and communicate with referral sources and primary care for progress and transition planning.
• Assist in scheduling treatment and arranging transportation; provide crisis intervention as necessary.
• Develop individualized treatment plans and advocate for clients/families to connect with community resources.
