Job summary

A&C Adult and Child Health, Inc. is seeking an experienced clinician to coordinate services for patients receiving Medication Assisted Treatment. The role involves reviewing assessments, planning transitions, and communicating with primary care providers to ensure holistic care.

You will advocate for families and connect them with community resources. The position requires knowledge of case management in mental health and substance use disorders, plus documentation that meets program standards.

Qualifications

• Bachelor’s degree in human services fields preferred.
• Experience in mental health or substance use disorder settings.
• Knowledge of case management and rehabilitation methods.
• Ability to document services per standards (KDHE/JJA).

Responsibilities

• Coordinate outpatient services (Mental Health and/or Substance Use Disorder) as needed.
• Review assessments and discharge planning to identify clinical and social needs.
• Communicate with referral sources and primary care about progress and planning.
• Assist in scheduling treatment and arranging transportation when needed.
• Develop individualized treatment plans and monitor progress.
• Advocate for families and connect them with community resources.
• Ensure chart accuracy and complete required trainings within set timelines.
