Job summary

Colorado Coalition for the Homeless is seeking an Addiction Specialist Coordinator to integrate SUD treatment, housing resources, and recovery supports. You will coordinate care across hospital, ED, outpatient, home health, nursing, psychiatry, and case management to promote continuity and stabilization.

Embedded in the SIPPRA team, you will help launch a Contingency Management program, develop care plans, track progress, and advocate for clients while maintaining accurate documentation in the

Qualifications

• High School Diploma or GED required.
• Bachelor’s degree in Psychology, Social Work, Human Services, Criminal Justice preferred.
• Combination of education and experience may substitute for position requirements.
• Colorado driver’s license within 30 days of hire (SSH C exception).

Responsibilities

• Coordinate comprehensive care for clients with Substance Use Disorder (SUD) across behavioral health, medical, psychiatric, and case management services.
• Facilitate access and continuity of care by coordinating referrals and transitions from hospitals or detox programs.
• Support MAT by collaborating with prescribers, nurses, and care teams for adherence and engagement.
• Implement and monitor Contingency Management initiatives to support recovery.
• Provide client-centered care management focused on stabilization, housing retention, and long-term supports.
