Job summary

Banner Health seeks a Behavioral Health Case Manager to provide comprehensive care coordination for patients at Banner Thunderbird Medical Center. You will assess care plans, coordinate services, and support families across diagnostics and age groups in a fast-paced environment.

The role requires a master’s degree in social work or related field, licensure and 5 years of clinical experience, with strong communication and leadership skills. Day shift 8:00 AM–4:30 PM; registry/per diem terms apply.

Qualifications

• Master’s degree in social work or related field.
• Licensure in social work, professional counseling or marriage and family (state required).
• Minimum 5 years of clinical experience with strong communication and critical thinking.
• Ability to work in an interdisciplinary team.

Responsibilities

• Coordinate care for patients across the health care continuum.
• Assess plan of care, implement and monitor utilization of resources.
• Collaborate with physicians and payers to meet regulatory requirements.
• Lead discharge planning and educate the health care team on case management.
