This role provides comprehensive care coordination, assessments, and utilization of resources to meet patient needs in a compassionate, safe environment.

The position supports collaboration with physicians, payers, and care teams to ensure appropriate utilization and discharge planning, with emphasis on effective communication

Qualifikationen

• Requires master’s degree in social work or related field or equivalent experience.
• Requires a proficiency level typically achieved with 5 years clinical experience and as required by state law, a licensure in at least one of the following categories: social work, professional counseling or marriage and family.
• Positions providing direct services to substance abuse program patients must also possess board licensure in addictions as required by state.

Aufgaben

• Coordinate care across the health care continuum to optimize outcomes.
• Provide assessment, psychoeducation, and psychotherapy to individuals, families, and groups; coordinate all aspects of patient care.
• Lead process improvement activities for populations of patients to achieve better outcomes.
• Evaluate medical necessity and promote collaborative care with physicians, payers, and other health care team members.
• Develop and manage discharge plans; communicate the plan across the continuum of care.
