Job summary

Qualifications

• 2+ years in behavioral health, social services, or human services.
• 2+ years using personal computers and MS Office Suite applications.
• 2+ years experience in children’s mental health or related public sector field.
• 2+ years in case management, family counseling, or child development.
• 2+ years familiarity with Ohio delivery systems and local networks.

Responsibilities

• Evaluate referred members' needs, eligibility, and plan an actionable care approach.
• Coordinate care plan activities and monitor progress with care teams.
• Collaborate with Medical Directors and other programs to access required services.
• Engage with Family Teams, escalate quality issues, and support member involvement.
