Qualifications

• Must reside in Danville, Virginia or Surround Areas.
• Willingness to travel up to 75% with mileage reimbursement.

Responsibilities

• Collaborate to inform the case management process and facilitate healthcare outcomes.
• Evaluate members’ needs, eligibility and plan appropriate care resolutions.
• Coordinate and implement care plan activities and monitor progress.
• Ensure quality of care through regulatory and accreditation guidelines.
• Use motivational interviewing to engage members in healthy choices.
