Legacy Health in Portland, OR is seeking a Behavioral Health Assistant to join an integrated care team dedicated to compassionate patient support in a behavioral health setting.

The role collaborates with a multidisciplinary team, assists with ADLs and safety, performs basic clinical tasks under an RN, documents observations in EPIC, and helps with discharge instructions while maintaining a safe, therapeutic environment.

Qualifikationen

• One year of clinical experience in a behavioral health setting; two years preferred.
• BLS for Provider certification required.
• Acute care experience preferred.

Aufgaben

• Collaborates with the multidisciplinary treatment team to support patient care in alignment with the unit's treatment model.
• Administers prescribed nursing interventions under the direction of an RN.
• Collects and documents patient data, including vital signs, as directed by RN.
• Assists with transportation of patients throughout the hospital.
• Provides support with activities of daily living (ADLs) such as toileting, hygiene, transfers, and cleaning up bodily fluids as needed.
• Delivers comfort measures including patient positioning and providing blankets/pillows.
• Ensures patient safety through restraints, fall prevention, and environmental hazard checks.
• Applies proper body mechanics during positioning, transfers, and ambulation.
• Performs basic tasks like EKGs, pulse oximetry, and point-of-care labs as directed by RN.
• Assists with discharge instructions and process under RN guidance.
• Communicates clearly with patients, families, EMS, and hospital staff.
• Maintains a therapeutic and safe behavioral health milieu.
• Accurately documents behavioral observations in EPIC.
