You will effectively communicate the patient's condition and significant changes to the assigned supervisory nurse and inform the caregiver of any issues. You'll be responsible for all practices and duties within the Hospice Aide scope of practice as outlined by the state.**MAJOR AREAS OF RESPONSIBILITY:**Patient Care: Provide care that includes baths, back rubs, oral hygiene, shampoos and changing bed linen as often as assigned. Take and record oral, rectal and axillary temperatures, pulse, respiration and blood pressure when ordered with appropriate completed/demonstrated skills competency. Assist in dressing and undressing patients as assigned. Assist in meal prep and feeding patients as assigned.Extended Services: Assist in ambulation and exercise as instructed by the primary care nurse or therapist. Perform range of motion and other simple procedures as an extensional therapy service as ordered with appropriate completed/demonstrated skills competency.Communication: Maintain good relationships with the patient, the patient’s family, and staff. Reports any changes to the patient’s medical and living conditions to the supervisor or the primary nurse.Minimize Patient Risk: Meet safety needs of patient and using equipment safely and properly (foot stools, side rails, Oxygen, etc.).
