Solari, Inc. is hiring a part-time 211 Program Specialist in Arizona to respond to community calls, identify needs, and connect callers to resources through virtual case management. The role requires documenting needs, referrals, and outcomes, with bilingual Spanish being a plus.

The position starts with full-time training hours before transitioning to part-time hours. Applicants must reside in Arizona and be available for onboarding and ongoing shifts.

Qualifications

• 0–2 years of experience in health and human services, public health, or healthcare.
• GED/HS diploma required.
• Proficiency with Microsoft Office, EHR systems, and basic computer navigation.

Responsibilities

• Respond to calls to identify needs and provide follow-up with connections to resources.
• Document identified needs, referrals, and outcomes.
• Provide high-quality confidential information, referrals, and advocacy for health and social services.
• Document all interactions in the web-based software systems.
• Maintain confidentiality and professionalism; follow recognized standards.
