Job summary

Elite Home Health values compassionate, patient-centered physical therapy services delivered in patients' homes across Florida. The PT will evaluate mobility, plan and implement therapies, and collaborate with an interdisciplinary team to improve safety and independence in the home setting.

The role emphasizes autonomous practice within scope of Florida licensure, with supervision of PTA and Home Health Aide as needed, and ongoing documentation to meet Medicare and agency policies.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current State PT license in good standing (Florida).
• Minimum of one year of experience in physical therapy; home health/acute care preferred.
• Current CPR certification required.
• Valid driver’s license with reliable transportation and auto insurance.
• Ability to pass background screening and compliance requirements.

Responsibilities

• Perform initial and ongoing patient evaluations to assess mobility, strength, balance, endurance, and functional limitations.
• Develop, implement, and update individualized physical therapy treatment plans.
• Provide skilled physical therapy interventions in accordance with physician orders and care plans.
• Supervise PTA and Home Health Aide as required by plan of care and policy.
• Educate patients and families on therapy goals, home safety, and mobility strategies.
• Maintain timely and accurate clinical documentation and compliance with Medicare/state requirements.
