Job summary
Elite Home Health in Florida is hiring a Physical Therapist to provide skilled services in patients’ homes across Ocala, Leesburg, and surrounding areas. The role requires evaluating and treating patients within their homes, developing individualized plans, and coordinating with physicians and the care team.
Florida PT license, CPR certification, and at least 1 year of PT experience are preferred. A valid driver’s license and reliable transportation are required to cover the coverage area.
Qualifications
- Graduate of an accredited Physical Therapy program.
- Current Florida PT license in good standing.
- Minimum 1 year of PT experience preferred.
- Current CPR certification.
- Valid driver's license, reliable transportation, and auto insurance.
- Ability to meet all required background and compliance requirements.
Responsibilities
- Perform physical therapy evaluations and develop individualized treatment plans
- Provide skilled therapy, including therapeutic exercises, gait training, balance training, and mobility interventions
- Educate patients and caregivers on exercises, safety, fall prevention, and assistive devices
- Monitor patient progress and adjust treatment plans as needed
- Complete timely and accurate clinical documentation
- Communicate with physicians, nurses, and the interdisciplinary care team
- Supervise PTAs as required
