Job summary

Elite Home Health Group INC is seeking a licensed Physical Therapist to deliver in-home PT services across Florida. The PT evaluates mobility, develops plans of care, and leads therapeutic interventions in patients' homes, coordinating with the interdisciplinary team to improve safety and independence.

Responsibilities include patient and family education, documentation, and adherence to Medicare regulations. Strong communication and autonomy in a home health setting are essential.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current state Physical Therapist (PT) license in good standing.
• Minimum of one year of experience in physical therapy; home health, acute care, or rehabilitation preferred.
• Current CPR certification required.

Responsibilities

• Evaluate mobility, strength, balance, endurance, and functional limitations; develop and update PT plans of care.
• Provide skilled PT interventions and gait, transfer, balance, and mobility training.
• Educate patients and caregivers on safety, home environment modifications, and therapy goals.
• Complete timely clinical documentation and ensure Medicare/state regulatory compliance; communicate with the care team and supervise PTA when required.
