Job summary

Qualifications

• Current Ohio Occupational Therapist license.
• Current driver’s license and reliable transportation.

Responsibilities

• Provides service within the scope of practice as defined by state laws governing Occupational Therapy and in accordance with the plan of care.
• Evaluates patient’s functional status and needs, consults with physician in therapy plan of care.
• Observes, records, and reports patient response and changes to condition to nurse supervisor and physician.
• Conducts patient assessments evaluating function via diagnostic and prognostic tests.
• Function in home settings regardless of age, race, creed, color, sex, disability, or financial condition.
