Job summary

UnitedHealth Group in Ohio is seeking an Occupational Therapist for Home Health who will assess patient needs, evaluate functional status, and plan care across daily living activities and adaptive equipment. You will work with a healthcare team, coordinate with physicians, and provide care in patients' homes with productivity-based payments including per-visit and hourly rates.

Qualifications

• Current Occupational Therapy licensure in state of practice.
• Current driver's license and vehicle insurance or access to reliable transportation.

Responsibilities

• Provides service within the scope of practice as defined by state laws for Occupational Therapy.
• Evaluates patient's functional status and devices, coordinates with physician for plan of care.
• Observes, records, and reports patient responses and condition changes to the nurse supervisor and physician.
• Conducts patient assessments applying diagnostic and prognostic functional tests.
• Function effectively in any home setting regardless of patient demographics.
