Job summary

Midland Care Connection in Emporia, KS, is seeking a Licensed Physical Therapist to join the PACE program on a PRN basis. You will work with an interdisciplinary team to help older adults remain independent at home, evaluating mobility, designing individualized treatment plans, and educating caregivers.

This role emphasizes autonomy within an inclusive care model, focusing on safe home-based therapy, fall prevention, and coordinated care with physicians, nurses, and therapists.

Qualifications

• Licensed PT in Kansas is required.
• Graduation from accredited PT program.
• Experience with older adults/geriatric populations.
• Knowledge of rehabilitation principles, mobility training, and fall prevention.
• Ability to perform physical assessments and develop individualized treatment plans.
• Strong communication and documentation skills.
• Ability to work in an interdisciplinary healthcare team.
• CPR certification (or ability to obtain).

Responsibilities

• Assess and improve mobility, strength, balance for older adults.
• Develop individualized treatment plans for home-based care.
• Educate participants and caregivers on safe movement and fall prevention.
• Coordinate with doctors, nurses, occupational therapists and other team members.
